For that reason, Health Horizons Home Care performs one of the strictest screening processes in the industry so that you have the peace-of-mind necessary to develop a friendship with your caregiver:

* Each of our employees are fully bonded and insured.

* Our home aides are required to be current on all documentation, including CPR and state certification.

* Health Horizons employees receive year-round professional training at our on-site School of Health Sciences – Health Horizons Academy. All of our home aides are required to receive at least 12 hours of in-service education per year. These hands-on courses may include such topics as: Fall Prevention, Alzheimer’s Education, Infection Control,
Communicating with the Hearing Impaired, Diabetic Care and more!

Q: Can Health Horizons Home Care staff help with medications?

A: Yes, Health Horizons Home Care can administer medications (RN – Skilled Services). And the Health Horizons staff can always assist in reminding individuals to take their medications. In fact, medication reminders are one of our most requested services! We can also report any side effects and assist with pre-measured medication packages.

Q: Will I have a choice of who comes to my home?

A: Yes, we encourage your involvement in the decision-making process! And with our thorough screening and selection system, you can rest assured that we will do our best to take the guesswork out of matching the right caregiver to you and your family.

Health Horizons Home Care operates with a “customer comes first” philosophy, so your complete satisfaction is of utmost importance to us! For that reason, we always try to accommodate any requests you may have for a change in caregivers (as allowed by law).

Q: Will I or my loved one always receive care by the same caregiver?

A: At Health Horizons Home Care, our goal is to establish a long-term caregiving relationship with you or your loved one. Should you want to request another caregiver for any reason, we’ll search for a match until you are happy! We understand that a strong rapport and trust with a caregiver is vital to a successful home care experience.

If you or your loved one receives care for many hours a day or at different times of the day throughout the week, it may be necessary to schedule more than one caregiver in a given week. Should you require multiple caregivers, our goal is the same…we want you to be satisfied and happy. And we’ll do what it takes to make sure we achieve that goal!

Q: What are the rates and minimal hours for your services?

A: Despite its growing reputation for providing some of the highest quality services in Hampton Roads, Health Horizons Home Care continues to be one of the most competitively priced home care providers in the area. Therefore, we are not afraid to talk pricing with you!

Every family we serve receives a Custom Care Plan, specific to their needs and budget. Several factors help determine the exact cost:

* Number of hours of care per week
* Level of home care services received (non-medical, skilled nursing, etc.)
* Area of the city where services are provided (easily accessible or remote)

Our services are typically billed on an hourly basis. This makes it easy to make a Custom Care Plan for you. To discuss the exact rates for the services you may need, contact our office. We’ll set up an in-home consultation to begin discussing a Custom Care Plan and the rates for each service on your plan. We always provide a written rate sheet prior to initiating services. And rates will never be increased without at least thirty days’ notice.

The minimum number of hours per visit is two.

Q: Will Medicare or my insurance policy cover these kinds of services?

A: Medicare covers home health care that is temporary and part-time, but only if you meet the following requirements:

* Your doctor must say that you need the care. And your doctor must document that you need care at home and make a plan for that care.
* You must need skilled care or therapy (physical, speech or occupational).
* You must be homebound. “Homebound” means that leaving home is a major effort and you cannot do it without help. When you do leave home, it must be to get medical care or for short, infrequent nonmedical reasons such as a trip to get a haircut, attend religious services or go to adult day care.
* The home health agency must be Medicare-approved. For a list of approved agencies, visit www.medicare.gov or call 1-800-MEDICARE.

If you do meet the above requirements, Medicare may pay for these services on a limited basis if you are also receiving skilled care, such as nursing care or other therapy. However, Medicare does not pay for 24-hour a day home care, prescription drugs, meals delivered to your home, homemaker services (such as housekeeping, laundry and shopping) or on-going personal care.

When it comes to insurance coverage, all policies are different. Some companies provide allowances for home health care, but it is usually limited. If you are uncertain as to whether your insurance company provides coverage or reimbursement for these serves, you should contact your local insurance representative directly for answers.

When seeking financial assistance for these services, please note that Health Horizons has preferred relationships with many companies who either fully reimburse or partially reimburse for home health care. Typical accounts for reimbursement include reverse mortgage, health savings accounts (HSAs) and flexible spending accounts. We also work with reimbursement programs like the Veterans Aid and Attendance Benefit, which is a benefit paid in addition to the monthly pension paid to Veterans and surviving spouses who require our services.

We’ll arrange a time to visit you, your family and/or any authorized representative to fully discuss your particular situation and how Health Horizons may be able to help. Together, we’ll go over the care your family needs and the care we provide. We’ll answer all of your questions regarding services and costs. Then, we’ll begin putting together your Custom Care Plan and matching your loved one with the right caregiver. Usually, care can begin within a matter of 24 hours!

NOTE: Whenever possible, ask to meet with the prospective caregiver(s) before they are assigned to provide you with care. This will provide a good opportunity to interact with the caregiver prior to an assignment. If you are not satisfied with the caregiver, this is a good time to let the agency know.
